Types of Urinary & Fecal Incontinence
Problems with bowel and bladder control, regardless of the cause, can be an uncomfortable and embarrassing problem. Often, people suffer needlessly because they don’t know that there are medical solutions.
Bladder control problems are divided into four types. The most effective treatments depend on the type and cause of bladder problems.


Stress incontinence
Stress incontinence is when urine leaks because of sudden pressure on your lower stomach muscles, such as when you cough, laugh, lift something or exercise. Stress incontinence usually occurs when the pelvic muscles are weakened, for example by childbirth or surgery. Stress incontinence is common in women.
Urge incontinence
This occurs when the need to urinate comes on too fast -- before you can get to a toilet. Your body may only give you a warning of a few seconds to minutes before you urinate. Urge incontinence is most common in the elderly and may be a sign of an infection in the kidneys or bladder.
Overflow incontinence
This type of incontinence is a constant dripping of urine. It's caused by an overfilled bladder. You may feel like you can't empty your bladder all the way and you may strain when urinating. This often occurs in men and can be caused by something blocking the urinary flow, such as an enlarged prostate gland or umor. Diabetes or certain medicines may also cause the problem.
Functional incontinence
This type of incontinence occurs when you have normal urine control but have trouble getting to the bathroom quickly enough. This maybe caused by such underlying conditions as arthritis or other diseases that limit you mobility.

Incontinence: Tips for Keeping Comfortable

When bladder control problems occur, it can take a great deal of extra effort to keep comfortable. Worry about leaks and odor can discourage you from doing the things you enjoy. The first step in managing incontinence is talking with your doctor. Incontinence is not simply an embarrassing inconvenience; it is usually a medical problem.
A complete medical exam will reveal the cause of bladder and bowel control problems. Products that absorb leakage may not be your only option. In some cases medication, exercises and surgery can eliminate or reduce the problem. However, without the help of your physician, there is no way to know for sure.
Medical treatment may eliminate some, or all, of the problem. Most people still need some form of protection for accidental leaks. To find the right products, start by evaluating the amount of leakage.
One of the biggest problems related to incontinence is skin irritation and breakdown. Even with scrupulous attention to hygiene, fragile skin will be damaged if exposed to moisture and irritants in urine or stool for prolonged periods. It may seem counterintuitive, but water actually dries skin out, leading to itching and cracking. Be sure to use a moisturizer frequently. Many lotions and creams work as a barrier to seal the skin surface and prevent liquid from being absorbed. These products significantly reduce skin irritation related to incontinence, and can help prevent infection.
In addition to various undergarments and pads, keeping dry at night may require under pads for the bed. Layering absorbent and waterproof pads and bedsheets makes clean up easy. If a late night leak occurs, simply remove the top layer of damp pads and sheet, and another layer is ready underneath.

Incontinence is the inability to control bowel movements or urination. The loss of urine or loss stool can be embarrassing and stressful. While incontinence is more common in older people, illness and injury can cause this problem in people of all ages.

Fecal Incontinence
Bowel control requires three components. A functioning anal sphincter, adequate room in the rectum to store stool until it is formed, and the sensation that the rectum is full all play a part in preventing the leakage of stool.
The most common problem that causes fecal incontinence is damage to the anal sphincter, a specialized muscle that control bowel function. The normal process of aging can cause incontinence through a combination of these factors. Chronic medical problems like diabetes, multiple sclerosis and spinal cord injuries can cause this problem at any age. In women, muscle and nerve damage from difficult childbirth is one of the most frequent reasons for loss of bowel control. Interestingly, subtle injuries may not cause symptoms until many years later.

Urinary Incontinence
These are different types of urinary incontinence, but the causes of each are quite similar. As people get older, the muscles that support the bladder, called the pelvic floor, can become weaker. Constipation and loss of bowel sensation leads to a chronically full bowel and can impact bladder control. Neurological disorders can also cause problems with urinary control. Chronic urinary tract infections, diabetes or the effects of medications are another possible cause. Mobility problems can make it difficult to get to the bathroom quickly enough.
Gender related conditions may also contribute to poor bladder control. For men, enlargement of the prostate, or prostate treatment can interfere with urinary continence. For women, hormonal changes that occur with menopause may lead to loss of bladder control.

There are several different types of crutches:
Under-arm Crutches:
These are the most common type in the United States, and are used most often by people with temporary disability or injury. Placing the pads under the armpits and holding the grip, which is below and parallel to the armpit pad uses these. These are sometimes known as axillary crutches.
Forearm Crutches:

Conditions such as sleep apnea, chronic obstructive pulmonary disease (COPD) and acute respiratory failure are sometimes treated with CPAP positive airway pressure or CPAP. CPAP machine involves using a mask over the nose, mouth (or both) during the night. A flow of pressurized air is blown through the mask to keep the lungs ventilated.

Some patients, however, are not able to follow the CPAP treatment because of shortness of breath. In other words, they find it difficult to breathe out against the continuous positive pressure of the air in CPAP. The alternative is a bi-level positive airway pressure (BiPAP).

While CPAP provides one pressure, BiPAP provides two:

Higher pressure during inhalation (IPAP)
Lower pressure during exhalation (EPAP)

BiPAP has different benefits, depending on the condition of the patient. A patient with chronic respiratory failure, for example, benefits from the BiPAP because the IPAP unloads the diaphragm and makes breathing easier. In the same way, patients with obstructive sleep apnea adapt better to the IPAP and EPAP settings of BiPAP and achieve a better sleep.

Comfort is an important factor during the use of CPAP and bipap because it improves compliance—the number of hours per night that the device is used. The longer CPAP and BiPAP are used every night, the better results a patient will obtain. At the beginning of the treatment, some people may find the mask uncomfortable, but they must keep trying because it takes some time to get used to it. Fortunately, there are different types of mask available that will fit their particular face shape.

CPAP and BiPAP are only for treatment—they do not cure respiratory problems. Depending on the condition, a cure may require surgery or other medical interventions. CPAP and BiPAP, however, can eliminate the complications of respiratory problems, while avoiding high-risk interventions.

Some people stop breathing momentarily when they sleep. This is called obstructive sleep apnea syndrome. People with sleep apnea have narrow throats. Their muscles relax during sleep narrowing the throat even more and causing loud snoring. Sometimes, though, the throat is completely blocked and air can’t reach the lungs.

The periods when they are not breathing are called apneas and can last up to ten seconds. Trying to breathe again, they often wake from their deep sleep. Thus, people with sleep apnea don’t sleep well at night and are drowsy all day. Interestingly, they usually do not remember their apneas from the night before. For this reason, relatives and partners are the first ones to recognize the symptoms.

Sleep apnea is common in obese persons and can cause some serious complications such as hypertension, heart disease, stroke, arrhythmia, extreme drowsiness and even changes in personality. During apneas, the level of oxygen in the blood drops, which eventually may result in the right side of the heart failing.

Fortunately, sleep apnea can be treated. Doctors usually prescribe a mechanical device that makes breathing easier, such as positive airway pressure. CPAP involves using a mask over the nose, mouth (or both) during the night. A flow of pressurized air is blown through the mask to keep the lungs ventilated.
A related condition is called central sleep apnea. This type of apnea, however, is caused by a problem in the brain—and those who have it are gravely ill. Central sleep apnea occurs in people with brain lesions, or other conditions affecting the brainstem.
When the sleep apnea is not related to other diseases it is called idiopathic central sleep apnea. While people with other types of apnea may treat their related conditions to eliminate the apnea, those with idiopathic apnea must use the CPAP, every time they sleep.

If you snore loudly and are usually sleepy during the day you may have sleep apnea—a lack of breathing while sleeping because the muscles of the throat relax excessively, thus blocking the flow of air.

Although sleep apnea cannot be cured, it can be treated through a CPAP Continuous positive airway pressure. A mechanical device pushes air into the lungs through the mouth, the nose, or through ventilation tubes.

cpap uses different types of masks, depending on the way the user breathes during sleep:

Some CPAP systems include humidifiers to increase the comfort of use. For example, these systems minimize condensation by compensating for any changes in the temperature of the room. Also, they maximize humidity if there is a leak and humidity is lost. This is important because without the humidifier the upper airways loose their natural moisture, causing pain or discomfort.

Some models include an Ambient Tracking device, which continuously senses changes in room temperature and alters the heater plate temperature accordingly to minimize condensation and maximize humidity.

People with obstructive sleep apnea that have trouble adapting to the conventional CPAP therapy may try the bi-level positive airway pressure (BiPAP)therapy.
BiPAP delivers air just as CPAP does, with the added feature of increasing pressure during inspiration. Once the inspiration flow stops, the BiPAP returns to the CPAP pressure level. BiPAP makes breathing even much easier and increases the comfort of the user.
Sleep apnea causes unrestful nights for many people. By using CPAP or BiPAP every night—for at least five hours—people with the condition can sleep better and enjoy their waking hours while avoiding the risk and complications of apneas—such as heart failure, heart disease, stroke, arrhythmia, and hypertension.

Heating Pads

A heating pad is a device that is used to warm up parts of the body in order to manage and alleviate pain. Heat and cold therapies work in different ways and provide different benefits. That means that sometimes, knowing which is the best treatment to use can be confusing.
Both hot & cold therapies work with your body’s own physiological methods to reduce pain, stiffness and prevent further injury. When an acute injury occurs, the body’s response is to open the blood vessels in the area. Increased blood flow sends more oxygen, nutrients and cells that can fight damage or infection to the site. Increased blood flow also creates swelling, your body’s natural way of restricting movement so there is no further injury.
Heat Therapy
Any increase in temperature of the skin or deeper structures causes the blood vessels to open, or dilate. A greater amount of blood can flow into the area when the vessels are dilated. This increases the temperature in the area, allows more infection fighting cells to work, changes the amount of fluid that can flow through the blood vessels and raises the metabolic activity of the cells that surround the damaged tissues. Because heat causes increased blood flow, it should not be used for the first 24-48 hours after an injury.

Cold Therapy
Lowering the temperature around an injured area works in the opposite way. Blood vessels respond to cold by constricting or closing down. This vasoconstriction restricts blood flow to an injured area. If there is an open wound, it will help to limit the amount of bleeding. Although less blood can enter the area, the blood vessels in the skin can dilate and the skin may look redder due to reactive hyperemia. The overall result of cold treatment is a decrease in swelling and the metabolic activity of the local cells. Ultimately, pain relief occurs for a combination of these factors.
Immediate application of cold packs can reduce the pain and severity of injuries and burns. Cooling injured areas prevents swelling, or edema from occurring or becoming worse. If applied quickly enough, cold treatment can reduce or prevent the formation of blisters.
Effective Use of Heat and Cold Therapy

Applied heat and cold therapies can be very effective in controlling pain and speeding recovery from injuries or muscle and joint pain. To effectively use these treatments, just follow some simple guidelines.
Heat therapy
Heat therapy can be divided into superficial and deep modalities. Superficial heat therapies include heating pads, heat lamps, warm moist compresses and medicated creams or gels. Pain associated with muscle spasms, leg cramps, menstrual cramps and superficial thrombophlebitis responds quickly to heat treatments.
A physical therapist or medical technologist usually applies deep heat therapies. These include ultrasound treatment, electric stimulation, paraffin baths.

Cold therapy
The acronym “RICE” reminds us how to treat acute injuries like sprains, strains bumps and bruises. It stands for:

ICE
COMPRESSION
ELEVATION

By following these guidelines, most minor injuries to skin and soft tissue like muscles, ligaments and tendons can be minimized. The body’s response to injuries like this is to swell quickly. Swelling prevents injuries in two ways. First, increased pressure from body fluids compresses the area and makes the nerves more sensitive to pain. The highly sensitized nerves respond more easily to painful stimuli, discouraging you from putting weight on the area or moving it. Next, the swelling acts like a splint, and reduces mobility at the site of the injury.
Applying cold therapy to these injures short circuits the body’s natural swelling response. By closing down blood vessels to the area, less blood and fluids enter the area, thus less swelling can occur. With diminished swelling, there is less pain and stiffness. Recovery can progress faster with less loss of strength, and therefore, less rehabilitation. Compression of the injured area also helps to prevent excessive swelling. Elevation uses gravity to encourage blood and fluid to flow away for the injured area, also working to decrease swelling.

Contrast Therapy
By combining the beneficial effects of heat and cold therapy, more chronic conditions can be improved. This method is most effective for long standing pain and stiffness associated with arthritic problems and joint pain. Persistent swelling after an acute in jury such as a sprain responds very well to contrast therapy. To perform contrast therapy, simply apply heat for 5 minutes, then apply cold for about 5 minutes. Repeat the cycle for 20 to 30 minutes.

Uses

In the 1950’s the first medications to control high blood sugar, called sulfonylureas were developed. Second generation sulfonylureas are still used today as a mainstay of diabetes. Since then, many different glucose-lowering medications have been developed. These include meglitinides, biguatides, thiazolidinediones and alpha-glucosidase inhibitors. Since each of these medications works at a different place in the metabolism of Glucose, sometimes two or more of these drugs can be combined for better blood sugar control.
